Each time you go to the pharmacy, you should be able to present your ID card and the pharmacist will process your prescription through your healthcare plan. However, sometimes something goes wrong and you end up having to pay for your prescription in full and need to be reimbursed. If that happens, here’s what you need to do.
Log onto www.caremark.com with your username and password.
From the home page, select Plan & Benefits from the listing of options. In the drop down, you’ll see Submit a Prescription Claim. Follow the instructions on the page to submit your claim and make sure you have a copy of your receipt to upload to the site.
If you’re unable to do this online, you can find a paper claim form by clicking on Help Center at the top right navigation bar. Click on Payments and Billing, and from there open “How do I receive a reimbursement for a prescription filled at the pharmacy?“. The answer to this question will have a Print Plan Forms opportunity. Your claim form can be found there.
